There's this interesting error that occurs with Outlook is that sometimes the standard toolbar get corrupted and multiple standard toolbars will appear.

When this happen no matter what you do, it just keep coming back.

Here's the solution, goto the following directory

Windows XP
C:\Documents and Settings\%username%\Application Data\Microsoft\Outlook

Windows Vista/7
C:\users\%username%\Application Data\Microsoft\Outlook

and look this file outcmd.dat, rename it!

That's it, problem solved, your outlook will be back to normal!
